From the previous article, we have learned about the conditions for applying for a pharmacy practice certificate, today we will move to the step of preparing documents and procedures to apply for a pharmacy practice certificate.<\/span><\/p>\n

Dossier of application for a pharmacy practice certificate<\/b><\/p>\n

Pursuant to Clause 1, Article 3 of Decree 54\/2017\/ND-CP as amended by Decree 155\/2018\/ND-CP detailing the application for a pharmacy practice certificate:<\/span><\/p>\n

    \n
  1. An application for a pharmacy practice certificate shall be made according to Form No. 02 in Appendix I to this Decree;<\/span><\/li>\n
  2. Certified copy of professional diploma. For diplomas issued by foreign training institutions, they must be enclosed with a certified true copy of the equivalent recognition certificate issued by a competent authority in charge of equivalence recognition as prescribed in Clause 2, Article 18 of this Decree. ;<\/span><\/li>\n
  3. The original or a certified true copy of the certificate of practice as prescribed in Form No. 03 in Appendix I to this Decree. In case of practicing at many establishments, the practice time is counted as the total practice time at the establishments, but a certificate of practice time of each establishment is required;<\/span><\/li>\n
  4. In case the application for a pharmacy practice certificate has different scopes of activities and requires different practice periods and professional practice establishments, the dossier must contain a certificate of professional practice time and contents professional practice of one or several establishments that meet the requirements of each scope and position of practice. In case the scope of professional activities has the same requirements on practice time and professional practice facilities, a separate certification is not required for each scope of professional activities;<\/span><\/li>\n
  5. The original or a certified true copy of the certificate of examination results issued by the examination organization specified in Clause 2, Article 28 of this Decree, in the case of a pharmacy practice certificate issued by the form of an exam;<\/span><\/li>\n
  6. For foreigners and overseas Vietnamese who apply for a pharmacy practice certificate in the form of a dossier review, there must be documents proving that they meet the requirements for using the language as prescribed. in Clause 2, Article 14 of the Law on Pharmacy.<\/span><\/li>\n<\/ol>\n

    Procedures for applying for a pharmacy practice certificate<\/b><\/p>\n

    Pursuant to Clauses 1, 2 and 3, Article 6 of Decree 54\/2017\/ND-CP detailing procedures for granting pharmacy practice certificates:<\/span><\/p>\n

      \n
    1. An applicant for the grant, re-grant or modification of a pharmacy practice certificate shall submit an application in person or by post to:<\/span><\/li>\n
    2. a) The Ministry of Health, in case of application for issuance, re-issuance or modification of the pharmacy practice certificate in the form of an exam;<\/span><\/li>\n
    3. b) The Department of Health, in case of application for grant, re-grant or adjustment of contents of the pharmacy practice certificate according to the form of application review.<\/span><\/li>\n
    4. After receiving the dossier, the application-receiving agency shall return to the applicant for the grant, re-grant or modification of the pharmacy practice certificate a dossier-receiving sheet made according to Form No. this.<\/span><\/li>\n
    5. In case there is no request to amend or supplement the dossier, the agency that issues, re-issues or adjusts the contents of the pharmacy practice certificate shall:<\/span><\/li>\n
    6. a) Issue a pharmacy practice certificate within 15 days from the date on which the application is received; in case of refusal to issue a pharmacy practice certificate, a written reply clearly stating the reason;<\/span><\/li>\n
    7. b) Issue a pharmacy practice certificate within 5 working days from the date on which the application is received, in case the pharmacy practice certificate is revoked according to the provisions of Clause 3, Article 28 of the Law on Pharmacy; in case of refusal to issue a pharmacy practice certificate, a written reply clearly stating the reason;<\/span><\/li>\n
    8. c) Re-issuance or adjustment of contents of the pharmacy practice certificate within 5 days from the date indicated on the application receipt; in case of refusal to re-issue or modify the contents of the pharmacy practice certificate, a written reply clearly stating the reason.<\/span><\/li>\n<\/ol>\n

      In addition to the necessary conditions, the preparation of the dossier is equally important because if it is not fully prepared or there are errors, it will take a lot of time to complete the dossier.
